FORMS ESSENTIAL TO YOUR DAUGHTER'S ENROLLMENT COMPLETION

School Year 2012-2013

The following forms MUST be completed and returned

by August 1 for students starting in September 6, 2012:

2. Medical Forms - Domestic New Students Cover Letter

All U.S. New students are required to complete pages 1, 2, 3, 4, 5, and 6. Pages 7, 8 and 9 are only necessary if they are applicable to your daughter. Please note: a baseline TB skin test (PPD) is required for all first year students. These results must be within the past 6 mos. A front and back copy of your health insurance card is also required.

2A. Medical Forms - International New Students Cover letter

All first year international students are required to submit pages 1, 2, 3, 4, 5, and 6. Pages 7 and 8 are required only if they apply to your daughter. A TB skin (PPD) test is required for all first year students. Physical examination and TB test results completed no more than 6 months before arrival on campus. All international prescriptions/instructions for medications must be written in English. All international students are required to purchase the United Health Care insurance. Insurance cards will be sent directly to the Health Center. Our local health care providers are familiar with this plan and coverage.

2B. Medical Forms - Domestic Returning Students Cover Letter

All returning students are required to submit pages 3 and 4 each year. Pages 7, 8 and 9 are also required if they apply to your daughter. All students are required to submit an ANNUAL physical examination - page 3. IF your daughter is considered high risk and answers yes to any TB question, a repeat TB test is required. If your daughter is prescribed daily medication, please contact School Med to register each year. Pages 1, 2, 5, 6 and a copy of your insurance card should be submitted ONLY if there are changes from the previous year.

2C. Medical Forms - International Returning Students Cover letter

All returning International students are required to submit page 3 and 4 each year. Students returning from high risk countries are also required to have a TB skin test (PPD) or Chest Xray each year. Pages 1, 2, 5, and 6 are required ONLY if there have been changes from the previous year. International Students are required to purchase the United Health Care insurance each year. Insurance cards will be sent directly to the school.
